Good Vibrations Massage and Acupuncture Privacy Policy

Good Vibrations Massage and Acupuncture is committed to providing quality healthcare to its patients, so it is essential to collect personal information and create a file pertaining to the patient’s health.

Client files contain the following types of information:   

  • Personal details such as name, date of birth, phone number, email address and health fund details.

  • Health history

  • Notes made during the consultation

  • Reports from other health practitioners

 

Circumstances under which your information may be used or disclosed.

Your personal health and information is handled with the highest level of respect for your privacy. Your file will be accessed by your practitioner or another practitioner doing your treatment, in order to provide a health service to you and record information on the therapy you received. It may also be necessary for staff to handle and view your file for administrative purposes in which case privacy requirements will be observed.

Your personal information, health record may be collected, used and disclosed for the following purposes:

  • For accessing and communicating your information to other health practitioners or your doctor.

  • For follow up reminders

  • For health insurance and accounting purposes

  • For legal disclosure if required by a court of law (eg: subpoena, court order)

  • For practitioners working at our clinic

  • We may disclose your health information to a family member or carer in the event of an emergency.

If you have any concerns, or wish to restrict access to your personal health information please discuss these with your practitioner.

You also have the right to access your own personal records held by us and can do so by contacting our clinic and make arrangements for access. You may correct any errors in your personal information by sending a request that can demonstrate that the information is incorrect.  We take steps to protect your privacy and security by verifying your identity before granting access to your personal information.

 

Records are stored securely

We are committed to preserving the confidentiality of the information contained in your health record. We maintain that strict secure storage policies are followed in this practice. We maintain all records electronically in Cliniko. Cliniko stores your data encrypted in restricted facilities.

We use Mailjet to mail out promotions and clinic news. Mailjet stores your name and email address on secure data servers.

Your electronic records are password protected and only accessible by the staff at this clinic. Staff members require 2 Factor Authentification for access. When a paper form has been filled out it will be uploaded to Cliniko and shredded immediately. Each member of staff is aware of the principles and importance of confidentiality.

`Should you have a query or complaint in relation to the privacy policies we have in place at this practice, please do not hesitate to contact the practice manager. We undertake to address your complaint within 30 days of receipt of the complaint.
